The provider will claim travel charges through the NDIS depending upon the distance traveled to provide a session in the community. This is followed as per NDIS guidelines for service providers.
Care Plan from GP
The sessions are provided through the referral from the GP using a Chronic Disease Management Plan (CDMP) also known as an Enhanced Primary Care Plan (EPC). The family is eligible to receive a rebate for five sessions through this care plan.
